A Welcome Letter

Welcome to the Community Health Centers Foundation website!   As a newly formed organization, we'd like to take a moment to introduce ourselves to you.

Our Initial Vision:

The Vision of the Community Health Centers Foundation is to improve the health of all Central Coasts residents by assuring their access to affordable, high quality health care.

Our Initial Mission Statement:

The mission of the Community Health Centers Foundation is to increase the ability of the Community Health Centers of the Central Coast to deliver quality health care at affordable costs and reduce health disparities through innovative approaches which empower our community members to choose healthier lifestyles.

Where appropriate the Foundation will seek funds to operate services and programs that complement the mission of CHC but are outside CHC's scope of services. 

The Foundation will be guided by the following values:

RESPECT:  At the center of the CHCF's efforts are to ensure that the people we serve will be offered a positive experience that health care is accessible to all by recognizing the diverse cultural, language, economic, educational and health status needs of those we serve.
 

AFFORDABIILITY:  CHCF will seek opportunities to strengthen CHC's delivery of health care by creating funding structures that allow identified community health needs to be prioritized and addressed in a financially-sustainable method.
 

CATALYST:  CHCF will be a catalyst for change in the newly designed health care system, using its role to stimulate new strategies for providing high-quality, affordable health care, promoting prevention and wellness, and reducing health disparities.
 

TRUST:  CHCF will earn the community's trust through its commitment to accountability, responsiveness, transparency, flexibility, reliability, and cooperation.
 

PARTNERSHIP:  CHCF welcomes partnerships, and its efforts will be guided by working with community partners, patients, providers, employers, government partners and other stakeholders.
 

RESULTS:  The impact of CHCF will be measured by its contributions to CHC's efforts to expanding coverage and capacity to meet the health care need of our community members, improving health care quality, promoting healthier living habits and health equity to all community members.
